2. Services Provided
Mental Perk Therapy offers professional mental health services, including but not limited to individual, family, and group therapy sessions. All services provided are confidential and delivered by licensed mental health professionals.
3. Client Responsibilities
Clients are expected to:
- Provide accurate personal and health information.
- Attend scheduled sessions on time.
- Give at least 24 hours’ notice for cancellation or rescheduling of appointments. Missed appointments or late cancellations may result in a cancellation fee.
- Follow through with the agreed-upon treatment plans and communicate any concerns regarding the process.
4. Payment and Fees
- Fees are due at the time of service unless otherwise agreed upon in writing.

- In the event of non-payment, we reserve the right to terminate services.
5. Cancellation Policy
A minimum of 24 hours’ notice is required to cancel or reschedule any session. Failure to do so may result in being charged the full session fee.

All client information is kept confidential according to HIPAA guidelines. However, confidentiality may be breached in the following situations:
- If there is a risk of harm to yourself or others.
- If child or elder abuse is suspected.
- If required by law (e.g., court orders).

7. Termination of Services
Either party may terminate therapy services at any time. Mental Perk Therapy reserves the right to terminate services for non-payment, violation of these Terms and Conditions, or other reasonable reasons as determined by the therapist.
8. Limitations of Liability
Mental Perk Therapy and its professionals are not liable for any outcomes resulting from the use of our services beyond the therapeutic environment. Therapy is a collaborative process, and results may vary based on individual circumstances.
9. Emergency Situations
Mental Perk Therapy does not offer emergency services. In the event of a mental health emergency, please call 911 or go to your nearest emergency room.
10. Changes to Terms
We reserve the right to modify these Terms and Conditions at any time. Clients will be notified of any significant changes before they take effect.
